DESIGN THE LAYOUT FOR A MULTIPLE CHOICE BOOK

I need a design and layout to be created for a book that will include multiple choice questions and images. This design would be in a WORD DOCUMENT and will be used as the template for creating an amazon book. So the design should be attractive and eye catching. I will need a design for the chapter pages and for the inner pages, cover design NOT needed, so it is mainly a matter of coming up with a pleasing layout. . Each page will have 5 questions, each question will have 4 choices and answer key at the bottom of the page. Some pages will have images along with the question , in which case, such pages will hold only two questions. So i will need 3 template pages: 1) chapter heading page 2) page with questions, answers and key 3) page with questions, images, answers and key.
