App for birthday party

I need an app that the user can access from their mobile phone through a QR code. In this app, I (as administrator) must be able to add a photo and text as a welcome presentation. The user who accesses the app through the QR code must enter their name and can upload photos, videos and text. Then, by pressing a button, the files and text will be uploaded to the cloud. In the cloud, an individual folder must appear for each user who has signed up.
