SIMPLE PAID TEST Azure Node Voice Agent MVP for big project

I want a minimal voice agent built with Node.js on Azure and connected to Twilio Programmable Voice. When I call the Twilio number, it should answer with a short friendly greeting, ask for my name and the problem I’m calling about, record up to 10–15 seconds, use speech recognition to extract the information, then ask: “What time are you free next week?” After I reply, the agent should acknowledge and say it will inform the doctor. A lightweight log (name, problem, preferred time, timestamp, and Twilio Call SID) must be saved in persistent storage (Cosmos DB, Table Storage, or any simple durable option). Deliverables: a live Azure deployment I can call and test, a clean GitHub repo with the Node.js code and Twilio webhook, and a short README explaining setup, environment variables, and redeployment steps. If everything works well, I have more voice and AI automation projects and would like to continue long-term.
