Help me to understand Aviation Safety and ability to show me how a case study incident should be analyzed.
Scope of Work:
The safety event that occurred on January 15, 2009. US Airways Flight 1549, in the initial climb after takeoff from New York City's LaGuardia Airport, struck a flock of Canada geese and lost all engine power. Must know system safety and fail-safe operations, as they relate to the actual incident.
In addition, relate the actual incident to the 5-step FAA safety risk process or the 8-step system safety process of the MIL-STD-882E (DOD, 2012 (Links to an external site.)) as if you were designing the failed functionality/components (engine power) of the overall system (airplane).
