I'm a safety consultant in Australia looking for someone to build a mobile-friendly risk assessment form system for tip trucks and earthmoving machinery.

It must include:

Form interface (Glide, Typeform, or Jotform) with tick-boxes, signatures, and photos

Risk scoring per item with before/after status

Payment integration (PayPal preferred) — report only emailed once paid

PDF output (with my branding)

QR code system: printed stickers link to the live/expired report

12-month expiry with optional CRM tracking and email reminders

I'm open to Airtable or Glide-based solutions.
