Construction Inspection Smartsheet Dashboard

I need a Smartsheet environment that streamlines how contractors request inspections and how that information rolls up to stakeholders. Contractors must be able to open a simple form that captures three things—inspection details, any supporting documents or images, and their contact information. As soon as they hit “submit,” the inspection team should receive an automated email so nothing slips through the cracks.
From there, the team will record a pass or fail outcome for each inspection. Those results, along with key project milestones, have to feed a live dashboard that instantly shows inspection status (pass/fail counts and trends), milestone progress, and an overall project status indicator. The display needs to be clear enough for executives to gauge project health at a glance without digging into the underlying sheets.
Please build or configure:
• A Smartsheet intake form with the required fields and file-attachment capability
• Automated email workflow that alerts the inspection team on every new submission
• Data sheet(s) to log inspections, milestone tracking, and formulas or reports that link them
• A visually concise dashboard featuring charts, progress bars, and a single overall-health metric
