patio slab certify report for city permit

I need someone to help me to get the permit with any city within the Dallas FortWorth (Texas) area.
(The city requires the stamp from the Engineer to apply for the permit).
1) I need help to draw the plan for the stand-alone Pavillion like the pictures attached (no fireplace)
2) I need the patio slab certify report for the city permit (for this project is for city of Irving, Texas)
