Design of Aluminum Folding Door Sign Off Forms

We are a door fabricator and make every door custom to our clients opening. In our process we need to confirm all details with a client before fabrication begins.

We need a one-page Word document to serve as our "Sign Off Form" for our exterior folding doors and windows. This is just a form we use for our customers to sign off on the details of every door before we start fabrication of the door. I've attached an examples of what we are currently using. We need a cleaner more professional look than we're currently using. The elevation drawing could be a "AutoCad" draft style or a 3D rendering look.

We're looking for pre made forms we can simple pick the configuration for any given project and plug in the measurements, colour, date and fields for the customer to sign and date, etc. (see examples attached)

Your job will be to:
- Include all the possible configuration options for our Aluminum Folding Doors and Windows. (see attached)
- Then we simple plug in the opening size measurements, frame size measurements, site address, name and contact number, purchase order number, colour selection.
Ideal Skills and Experience:
- Excellent design and layout skills
- Experience with creating forms or templates
- Understanding of construction or architecture a plus
