Forma Pauperis Written For Federal Civil Case for Discrimination

Currently have a Federal Employee EEOC in Final Agency Decision. Was currently in EEOC Hearing but after EEOC Judge sanction the agency after not attending EEOC and additional sanctions. A Default Judgement was requested by me but judge refused. Therefore asked for FAD. Now I’m sure it will return no discrimination found. Preparing for Federal Civil Court. I need an forms pauperis written up due to my social security status etc and mental health not fit to present my own case.
