Help finish Kentico 9 Form

We are still running Kentico 9 as we prepare to switch platforms. We have a survey form built, but it needs to have user profile information added to it so that we may easily associate responses with our users. I have some ideas about how to do this, but I am not as familiar with Kentico as the platform to which we are migrating. I would appreciate help with finalizing the form.
