Create .pdf from web form - ASP.net

Need to turn the attached excel file into a web form, it has 75 questions on it. The answers will then be used to output into a pdf report. An example PDF to use as the template is attached.

- Excel file - turn this into a web form capturing the answers.
- PDF file - this is the template/final output to use as a guide.
The web form will have a button at the bottom, when submitted it will create the pdf output.
The web form needs to be in asp.net.
