Budget: $10 – $30 USD

Choose ONE (1) activity as your social responsibility project. This project can be done individually or in a group of three to four people depending on the size of your chosen project. Ensure that your chosen project is doable and manageable (individual or in a group) within the time given.

The items must be included in the report: [Minimum: 15 pages]
• INTRODUCTION:
o Background of the project.
o Reasons for choosing this particular project.
o The parties that will benefit from this project.
• THEME:
Example: Cleanliness, Love, Community Collaboration, Information Literacy, Health, Mental Health and others.

• PROJECT NAME:

• PERSONNEL INVOLVED:
o The names of the persons actively involved in the project.
o How every member communicates with each other (students are required to take and include a screenshot of any and all interactions in the appendix section).

• OBJECTIVES OF THE PROJECT:
o The outcomes to achieve from the project.
o Make sure a feedback form is prepared to assess the success of the project that has been carried out (make sure students include the findings from the feedback form provided).
• PLANNING OF THE PROJECT:
o The aspects involved in the planning phase.
o Inform how the distribution of activities or responsibilities is done.
o Inform how you contact relevant parties, such as non-governmental organisations, responsible officials, school principals, department managers and others to carry out your projects (make sure to include screenshots of texts/screenshots of emails/pictures of meetings or discussions that took place).
o If any help or donation is needed, how this is communicated with relevant parties (include screenshot of this communication; fliers, text messages, Facebook posts, etc).
o If donation is collected, present the breakdown of the amount and its uses.
o Communication – Must include the way and tools used to disseminate about the project to the community, i.e. fliers, text messages, Facebook posts, etc).

• EXECUTION OF THE ACTIVITIES IN THE PROJECT:
o Detail all of the activities you have planned according to the itinerary prepared.
o If the project is done in groups, include the distribution of responsibilities for all members during the execution of the project. For example, Student A ensured that all the guests to the online sessions provide their feedbacks through the form prepared.

o What were some of the issues or problems you encountered throughout the planning to the execution of the projects? How did you solve the issues?

REMINDER:

1. Take pictures of the planning or activities done in the project and include them as the appendix in your report (COMPULSORY).
2. Also make sure that any request for permission letters or relevant documents are inserted in the appendix section too. The request for permission letter or relevant documents should have the name and contact information of the person in charge. (COMPULSORY).
3. The pictures included must clearly show the student/s FACE/S (COMPULSORY).
4. Any report sent that does not have pictures or relevant documents will not be accepted.

• Use New Times Roman font, size 12.
• Please justify the paragraphs in your report. As a university student, you should know how to justify your paragraph.
• Use the cover page included in this document in your report.
