I need a Poet to rewrite the attached euology

The anniversary of my Mums passing is in 4 weeks and I would like to mark the occasion with a poem derived and inspired from the eulogy. I have no set expectations for the poem.
Ideally the length should be comfortable to read on an A5 page. I would like to add it to a card for a frame and give to family members.
I should add UK or Ireland based person as the context of Irish Mother important.
