Wix Intake Forms & Relational Backend
Budget: $30 – $250 USD
I need to add a set of intake forms to my Wix site and have every submission stored in a relational database inside my Google Workspace environment. The forms only have to collect information—no file uploads or payments—yet the data must be structured properly so I can run queries later.
What the forms must capture
• Contact details: email address, phone number, physical address
• Personal identification fields I specify
• The service the visitor is interested in
I will provide a full schema: table names, field names, and the relationships between them. Your job is to build the Wix forms (Velo, Corvid, or another suitable method), connect them to the Workspace backend (Google Sheets as relational tables, Apps Script, or Cloud SQL—whichever you recommend), and validate each field to avoid duplicates or bad formatting.
Acceptance criteria
• Live forms embedded on the site, styled to match the current theme
• Data saved to the correct tables and columns on every test submission
• Auto-reply email to the visitor plus an internal notification to my team
• Brief hand-off document showing the code snippets, API connections, and how to add new fields later
