Shopify Order Fetching Form

I need a Zapier solution to create a form that retrieves previous orders using a customer's email. The form should be embedded on my Shopify website.

Requirements:
- Use Zapier integration
- Fetch data from Shopify
- Embed form on Shopify site or possibly another site (Odoo). I would just need the code for this.
- User-friendly interface
