Sequential Jotform + WooCommerce Checkout Automation

Jotform + WooCommerce Integration Needed

We are looking for an experienced WordPress / WooCommerce developer with strong Jotform experience to build and integrate a series of sequential Jotform PDF forms into our WooCommerce checkout flow.

Project Summary
Customers must complete several Jotform PDF forms after confirming their cart and before accessing the checkout page. Each form must be completed in order, with the final form triggering emails and redirecting the customer to checkout.

Key Requirements
- WordPress website using WooCommerce
- Recreate existing Adobe PDF forms as responsive Jotform PDF forms
- Multiple forms completed in sequence (no skipping)
- Each form includes an “I Agree” checkbox and navigation
- Conditional logic required for medical forms: When specific medical conditions or answers are selected, a Medical Clearance Form must be displayed and the customer must be able to download the Medical Clearance form
- Final form includes:
Name (First & Last)
Signature
Date
Final “I Agree” button
- On final submission:
1. Send two emails (admin + customer) with PDF copies of all completed forms
2. Redirect customer to WooCommerce checkout page

Desired Workflow
Customer clicks “Register and Checkout” on Cart page
Customer completes Form 1 → Form 2 → Form 3 → Final Form
Final submission sends emails and redirects to checkout
