Jotform-Healthie Intake Automation Setup

I need an engineer familiar with Jotform, Keragon, and Healthie EMR to wire an end-to-end automation that fires the instant a patient finishes our online intake form.
Workflow
• Trigger: Jotform form submission
• Middleware: Keragon for field mapping, transformation, and secure hand-off
• Destination: Healthie EMR, creating a new client record and attaching the full intake
Scope of data
Personal information—name, age, phone, email, address—plus every response contained in the intake forms must arrive in Healthie exactly as it appears in Jotform or pdf submission No manual touch points.
Key tasks
1. Connect the existing Jotform to Keragon and authenticate both systems.
2. Map each intake and demographic field to its matching object in Healthie.
3. Build error handling and simple logging so failed transfers alert us by email.
4. Run test submissions with me, confirm records populate correctly, and document the final setup so we can maintain it.
Acceptance criteria
✓ New patient submissions automatically create or update a Healthie client.
✓ All specified fields display in the correct sections of the chart.
✓ No duplicate records on repeat submissions.
✓ A concise hand-off doc (screenshots or loom) explaining the flow, field map, and how to edit it.
