We currently have a PDF contract and want to create a simple, professional system where our technicians can:

Fill out a customer form on an iPad (on-site)
Automatically populate a contract
Send it via DocuSign for signature
Receive a signed copy instantly
Scope of Work:

We need a freelancer to:

1. Create a JotForm

Customer information (name, address, phone, email)
System details (monitoring plan, pricing, activation fee)
Payment info (credit card / ACH fields)
Checkboxes (terms, auto-pay authorization)
Mobile/iPad optimized layout

2. Integrate JotForm with DocuSign

Connect form fields to our contract
Auto-fill contract fields
Trigger DocuSign after form submission

3. Setup DocuSign Template

Upload and prepare our contract
Add:
Signature fields
Initial fields
Date fields
Map all fields correctly

4. Workflow Setup

Customer fills form → DocuSign opens → signs
Signed copy emailed to:
