Candidate Data Collection Forms

I need a set of responsive online forms that capture three key pieces of information from each applicant—contact details, employment history, and their skills & qualifications. All submissions must flow straight into my existing HR system, so the form should post to its API (or another secure method you recommend) without any manual intervention.

Here’s what I expect as the finished deliverable:
• A branded, mobile-friendly form (or small multi-step wizard) hosted on my domain or delivered as embeddable HTML/JavaScript
• Server-side logic that validates required fields, handles file uploads for résumés, and forwards the data to the HR system’s endpoint
• A simple confirmation e-mail to the applicant plus an admin notification so we know each record arrived safely
• Clear setup documentation so I can redeploy or tweak fields later

If you have previous experience integrating forms with HR platforms such as Workday, BambooHR, or a similar REST/GraphQL API, please mention it; that will speed up onboarding. I’m ready to provide API keys, field mappings, and branding assets as soon as we start.
