Need a angular developer to fix problem in file upload

File upload is already implemented in my application (Angular 11 + .net core api) and files are getting uploaded . I need some expert to modify the code in the following way.
There is a module called Lab test in the application where doctor can prescribe different tests which is working fine .
I need file upload to be modified in such a way the files can be uploaded specific test and saved with name of test+patientid+date+time.
