Create AngularJS Form and backend to integrate DAO + Service to get data

The project includes creating UI for a form and write service/DAO layers. UI code already exists in other forms, code can be copied/reused. Once the data is entered in the form - Save and Cancel buttons functionality implementation.
I will walk you through the tables/.js files/controllers etc.
Mockups are available for the form. This is only one form.
