medical app hisory

I want an application that allows the user to sign in and create an account with his personal data then he can start storing his medical history data like (blood type, the dates for the last scans and pics for it, last check-up, what surgeries did he do, levels of sugar ......etc. and store this data in the cloud so I can access this data anytime as well. its some kind of project for my study
