Secret Shopper App for ios and Android

I have a secret shopper business. Currently the shoppers make notes on the phone and type everything up at home. I would like an app they can login to and fill the questions out on the fly, while on location. I have all the questions typed up. I need the app to act like an old mailto form that emails the info back to a database or email when finished.
