Modification for my Android App. Form generator. The web is done on php & Laravel and the mobile is done on flutter.

I need some modification to my app. How my app works it basically: A customer fill out a form on the app and then completed form will be send to my email automatically then I can open the automatically generated email and I can click on the link and then takes me to a page and opens up pdf format of the completed form.
Changes:
1. The final completed form does not look good at all.
2. The 'date" format on the form is England/Europe format(DAY/MONTH/YEAR), I want US format(MONTH/DAY/YEAR)
3. On the side of the page(when customer is filling out the form) I want it to say. Please go to the next page.
4.After completion of the form, It takes almost 4 minutes to FASTER PROCESSING
