Mobile Timesheet Forms Development

I'm in need of a proficient developer who can create mobile timesheet forms for both iOS and Android platforms. The forms should be user-friendly and efficient for employees to track their work hours, machine hours, and labor hours.
Key Requirements:
- The forms should be compatible with both iOS and Android platforms.
- The forms should allow employees to record their total hours worked, labor hours and machine hours.
Ideal Skills:
- Extensive experience in mobile app development, especially in creating forms.
- Knowledge of both iOS and Android development.
- Ability to design user-friendly interfaces.
Attached is the form I currently used. Currently I use paper form that gets filled out daily and turned into me daily. I would like to create a form on air table that I could have my employees fill out and submit daily. The problem I'm running into is being able to separate out equipment hours from labor hours.
It needs to be super simple with the ability to add features in the future if needed.
