Form Filling App for gathering data.

I would like an app that works on both Ios and Android devices. I want a relatively simple form which will be used to gather and store data from customer sites. There will be appoximately 30 fields that will be completed on each form. I'd like the app to be able to synchronise data to a cloud based storage drive. I'd like to be able to ad photos from the device camera and I'd also like to be able to save records and copy and edit them to save duplication of work.
