Flutter MS SQL Data Entry App

I need a Flutter application that interfaces with an MS SQL database. The app will have the following features:

- Login Page: User authentication with ID and password.
- Data Entry Form: Capture the following data:
- Data Entry ID
- Data Time of Entry
- First Name
- Last Name
- Photograph
- Employee No. (scanned via QR code, supporting both 1D and 2D barcodes)
- Geo Location of Data Entry
- Register
- Save all data directly to MS SQL database upon entry.
- Logout functionality.
- Session timeout for security.
