Create a smart-questionnaire app

I want to create an App or web App that sends a questionnaire to custommers without registration or downloading anything.

The questionnaire will change the questions based on an algorithm, and depending on what has been replied to former questions.

The app will create a pdf repport with all the data collected from the custommer.
