BOOKING APP for ANDROID and IOS and HUAWEI App gallery

i need my html booking form converted into an APP. for Android IOS App Gallery Amazon app etc but the main two are ANDROID and IPHONE.
its a simple form u just fill in a few fields and press submit dont even have to login
the form currently sends an email to the manager and inserts booking info into mysql db.
ill give u the restaurant name later for the app and logo we sort that out later
