Android Mobile App to Scan and store QR code Data

I am looking for a Freelancer who can help me create an Android Mobile App and Web for Reporting status that will scan and store QR code data. This app needs to be able to scan text data, health data, and geographical locations. The freelancer I choose will need to provide a detailed project proposal when applying for the job, as well as past work experience. My desired timeline for delivery is 2 weeks.
