Combine and Plot Functions in Mathematica

I have a Mathematica notebook that is nearly complete, but I need assistance with one specific plot. The plot consists of two halves, which I can currently plot separately. However, I need help combining these two halves into a single function for further plotting and calculations. The relevant plot appears at Out[456], which should be helpful for reference.
