Airtable questionnaire

I'm looking for a freelancer to help me create a questionnaire using Airtable. The questionnaire Airtable will be used to collect customer feedback as a link sent to them, so the freelancer just needs to create the 6 questions questionnaire. Some answers is to enter data and others to chose ready options to answer.
