Prepare projects for Agile continuos integration

One team I'm working with has problems delivering agreed tasks on time.

I'm looking for consultant to document two projects at the moment and create a process to on-board additional developers and engineers without a hassle.

Currently the projects are too dependant on current team and that must be fixed asap as projects' delivery is late, while the issues to solve are extremely urgent.

First project stack:
Ruby
Angular
PHP
Couple web servers, at least two systems
Git

Second project stack:
Wordpress Sage (JS, Blade, PHP)
