Auto Generating Contract

I am in need of a form to be built that is filled out by my sales representatives, and the answers on this form are converted to a PDF in a contract form, with fields for the customer to fill out and sign. The questions that our sales representative would fill out, would be things such as price, and terms that will be auto added to the PDF, and then some of the fields will need filled out by the customer such as address, name, and signature. I have all of the text, and a general layout for the contract, I just need it simplified, and something that will be easy to fill out on the go. We subscribe to Adobe, and can use acrobat, or adobe sign to gather customer info, however, I want it to be a simple questionnaire for our employees to fill out with the answers auto generating in the PDF. I do not care how it is created, it must be visually appealing though.
