PNG Table Formatting Needed

I need a two-page Table of Contents in a PNG file that I can insert into a book I am publishing. The software I am using only generates a TOC with chapter titles and pages (I want to include the references). The book is 6" X 9". Title font (CONTENTS) is Cinzel 33 point font, text should be EB Garamond 11 point font. Margins need to be 0.5" outside, 5/8" top, 1" bottom, 5/8" right gutter, 1" left gutter. First TOC page is a right side page, second is left (the TOC will extend to two pages).

I am also attaching a PNG of one contents page that someone created for my last book; the style should be the same for this new one with the lines of dots between the columns. I have attached a DOCX file with the titles, references, and page numbers in columns.

Finally, there is a PNG of the star logo that appears before each chapter title in the Contents. This will need to be scaled to the size of the contents font.

The final result should be two PNG images with the stars added to the left of titles and the dots between columns. I can insert the PNG images into Atticus.io.
