I need a multi-layer logo created in Photoshop from a photograph of an existing logo.

Requirements:
- Create a logo as a multi-layered Photoshop format from a provided image of an existing logo (image provided)
- The logo should include:
- Text
- Icons/Symbols
- Background elements
- The final product should have six or more layers for color separation
- The final image should be round (the photo is not perfectly round)
- The final image should be 300 dpi and 11 inches in diameter
