I have included several files. described below. This is a double sided one page document scanned that you need to convert to a digital version using photoshop or similar. final output file will need to be a pdf in layers with all boxes and text on separate labeled layers. i must be able to open in older photoshop version 8.0.
To start you will create a 300ppi document and then make the size to 8.5 x 11 inches. (the sizes of this document. Then import the raw file from below and then you will do your drawing over top. everything must be a 1:1 duplicate of this scanned image, the fonts need to be identical, and the background shading of the boxes should look the same as well. Then repeat that process one more time for the reverse side.
Attached files:
Raw front (scanned front image that you will make digital)
Raw Back (same as above but the back side)
Additonal digital files are available (KI-2024 back and front) if you want to use them and make modifications to make this quicker (this file was made digital last year and i have enclosed those files on layers )
