Revamp Interactive PDF Onboarding Form

Our current PDF onboarding form feels clunky, dated, and a bit intimidating for new clients. I want it to be something they can breeze through in minutes, so the goal is three-fold: give the document a cleaner, more modern layout, build in light interactivity, and make every page intuitive at first glance.

Design & layout
The file needs a fresh visual hierarchy—consistent spacing, clear section breaks, and typography that looks professional without feeling stiff. If you have a proven eye for page composition in tools like Adobe InDesign or Illustrator and can export crisp, accessible PDFs, that experience will come in handy here.

Interactivity
Instead of forcing people to type everything, I’d like the form to rely on intelligent checkboxes and radio buttons wherever practical (think policy acknowledgements, benefit selections, equipment requests). Every interactive element should work flawlessly in Adobe Acrobat Reader as well as in common browser-based PDF viewers.

Ease of completion
Field order, tab sequence, and logical grouping are crucial. When a new employee presses Tab, the cursor should jump exactly where they expect. Helpful inline hints or placeholder text are welcome, just keep them concise.

Deliverables
• A fully redesigned, fillable PDF (interactive checkboxes & radio buttons implemented)
• Original design file(s) so we can edit later if policies change
• Quick testing walkthrough or screen-share confirming that all fields, checkboxes, and radio sets behave as intended on both desktop and mobile PDF viewers
