Pensioner ID Card with QR

I need a printable identity card template for pensioners that looks official yet easy to reproduce at scale. The front of the card should feel traditional and formal, carrying our organisation’s crest and colour palette. Required visible fields are the pensioner’s name with a clear head-shot photo, address and contact number, plus date of birth alongside the pension ID.

A scannable QR code must sit discreetly on the reverse (or a lower corner if you think it works better) and be configured to store the same personal details rather than linking out to a website. Please embed the data in a compact, widely supported format such as vCard or a small JSON block so any standard scanner immediately reveals the information without an internet connection.
