PDF Form Update & QR Code

I have an existing form in PDF format that needs a quick but precise refresh. On the front, I want a clean-looking QR code that links to a specific page on our website; I’ll share the URL as soon as we start. The code hasn’t been generated yet, so please create it in a high-resolution vector (SVG or EPS) and embed it so it prints sharply.

Flip the form over and add a small section listing all of our locations; I’ll supply the exact addresses. Once everything is in place, deliver a press-ready, high-quality PDF (300 dpi, CMYK) plus the standalone QR artwork.
