Modern Fillable Home Certificate Design

I need a sleek, modern certificate that certifies a newly “hardened” home. The first version will act as a sales sample for the builder, so it has to look polished and professional while leaving room for later refinement. Once the builder approves the concept, we will iterate a second, final version that incorporates any feedback.

The certificate must be entirely digital and built around a fillable form field where the site supervisor can type the exact street address before printing and physically signing the document. A fillable PDF is the logical choice, but I am open to any format that achieves the same friction-free data entry and high-quality print output.

Core expectations
• Contemporary visual style that aligns with the “Modern” aesthetic selected.
• Clearly branded “Hardened Home” theme, including space for logos I will provide.
• One prominent fillable text field for the property address, locked design elements everywhere else so the look stays intact.
• Room for a handwritten signature from the builder plus the date.

Deliverables
1. Version 1 – presentation-ready fillable PDF (and the source file—Illustrator, InDesign or similar).
2. Version 2 – final polished certificate after feedback.
3. Simple instructions so office staff know exactly where and how to type the address before printing.
