Modern Certificate PDF Form Design
Budget: $10 – $30 USD
I’m putting together a sleek, blue-and-white course certificate and need a fully interactive PDF form. The layout should feel clean and contemporary, with space for five fillable fields—Candidate Name, Course Name, Certificate Number, date and a signature area that supports digital signing.
Use the name field font Edwardian Script ITC or Ballet for the name field; course name font can be anything suitable, signature field need to be a signature where one can write name and create signature or attach picture. The finished file has to be print-ready (high-resolution, CMYK) while also keeping the form logic intact so users can type directly into it or drop in an e-signature.
Deliverables
- certificate design to match company logo (logo will be provided)
• Editable, fillable PDF certificate in A4
• Source file from your preferred design app (Illustrator, InDesign, Affinity Designer or similar)
• One quick mock-up populated with sample data so I can confirm the field flow before final export
