Graphic Box Design for SMS Opt-in

I need a freelancer to create a nice graphic looking box titled at the top
"Receive Order Confirmation Via SMS Text Message" : it will have 3 keyable boxes that are free type for customer and labeled "First Name", "Last Name", & "Mobile Number" . then the verbiage will say " I agree to receive SMS text messages from Shop Home Services related to my order details, account updates, and special offers. I can opt out at any time by replying "STOP" to the SMS message. Carrier message and data rates may apply. Check with your carrier for details." This box should be about 3 inches tall by 4 inches wide with a Graphic border two tones of blue border so it looks nice, and it needs a nice size checkbox inside for the opt in to receive SMS messages box that can be checked or unchecked and that box should also be a thicker light blue check box that looks nice . I need to be able to put this box in emails we send out so we get approval via email to send the customer sms messages. This is TIME SENSITIVE! Please do not bid unless you can do this in the next 3 hours, but preferably sooner. There will be a nice tip if it can be done right way.
