Clutter Scale Flowchart and QR

I have the official Clutter Image Scale and need it distilled into a single, modern-looking flowchart. The chart should show only key reference photos, grouped exactly as 1–3, 4–5, 6–7, 8–9, and 10. The flowchart should be an easy to follow guide to help individuals accessing the flowchart when they need to seek help and make a referral.

Please lay the design out in a clean, minimal style with plenty of white space and consistent typography. I will supply the source document and photos; you decide which representative image best illustrates each band while keeping the file uncluttered.

Deliver the finished piece as a high-resolution PDF. In addition, generate a scannable QR code that links directly to this PDF (or to a shareable online version I can host). Provide the QR as a separate PNG so I can drop it into print or digital materials later.

If you work in Adobe Illustrator, Figma, or a comparable vector tool, that’s perfect—just be sure the PDF exports crisply and the QR code resolves correctly when tested on standard mobile readers.
