DTP Application Forms Design

I'm looking for a skilled designer to create professional looking application form. (realty sector booking form / India geo). These forms will be used for both online and print purposes, and need to be fillable electronically as well as printable.

Preliminary design of the document and all required contents is created and readily available using MS Word. The same needs to formalised / formatted / aligned / neatly arranged / organised using appropriate tools, make it user friendly, presentable.

Ideal Skills and Experience:
- Proficiency in design software (Adobe InDesign, Illustrator, etc.)
- Experience in creating user-friendly, professional forms
- Knowledge of digital form creation and compatibility

must be willing to support revisions (after current delivery) at later stages to meet other business needs (for a fee).
