I will need the Dropdown Items list to be updated on the document provided
each of the dropdown fields will need to match and allow user to enter custom text
- Payslips to be changed to “ Salary/wages – This would be most recent pay-slips covering the last 4 week period. Job letters are acceptable but must indicate net earnings within the last 4 week period.”
- Termination letter / severance letter
- WORC Registration
- Proof of scholarship
- Each loan selection should have (must show current payment due, balance owed and arrears if any) also to say financial institution
- Water statement to be changed to Water Utility Statement also to say (must be current month)
- CUC to be changed to Electricity statement also to say (must be current month)
- Marriage/Divorce Decree to be added
- Health Insurance Quote
- Schedule of Benefits
- Business License
