There are technical issues in a pdf form I need fixed. Example - greyed out data entry fields and validation button does not work.

I'm looking for someone to help me with a pdf form, as certain technical issues are preventing me from using it as intended. The particular issue I am experiencing is the data entry fields being greyed out (eg page 9) of attached, as well as the validation button not working properly (on page 3) after keying in required data into the fields and attempting to validate. The original purpose of the form was for data collection, so it is important that these technical issues are resolved quickly.
For now, I just need the existing technical issues with the pdf form fixed; I do not require any additional features or improvements added at this time.
1st file attached (FUND-ET_1.2) is the original unfilled pdf form and 2nd attached file (FUND-ET_1.2V3) is the form after I have filled up with details but technical issues started such as validation button not working and fields become uneditable.
