Single Page Fillable PDF Form for office work

I need a polished, one-page registration form turned into a fully interactive PDF. Every field on the page must be fillable: standard text boxes for names, checkboxes, a few dropdown list which can be added or modified.
To keep collected data clean, please apply basic validation so the phone field accepts only a valid number format and the event date field limits input to a proper date format (MM/DD/YYYY).
The completed file should open smoothly in Adobe Acrobat Reader and modern browser viewers, save entries, and allow users to print or email the filled form without losing their data. Deliver the ready-to-use PDF plus the source file you build from (InDesign, Acrobat, or similar) so I can tweak wording later if needed.
