I’m preparing a short, email-based questionnaire and need it turned into a clean, fillable PDF that respondents can complete and send back without printing.
Core requirements
• Interactive fields for Name and Address, plus Date and Signature.
• Room for the survey questions I’ll supply as soon as we begin.
• Smooth tab order and clear visual cues so even non-technical users can move through the form quickly.
• Signature box that accepts typed, drawn, or certificate-based signatures in Adobe Acrobat Reader.
• Fully compatible on both Windows and macOS (Acrobat and Preview).
