Create a professional fillable PDF Form from my Word Document (No auto-calculations)

Description: I have a 10 page questionnaire with 50 questions created in Microsoft Word. I need it converted into a clean, professional fillable PDF form.

This is a business self-assessment worksheet that customers will complete and email back to me, so it is important that the PDF looks professional and is easy to use.

This is not a complex project. It is mostly checkboxes and a few text fields.

What I need:
1. There are fifty questions. Each question must have three checkboxes labeled 0, 1, and 2.
2. After each section, there must be a text field where the user can type their section total. And a text field for them to type any notes.
3. At the end, there must be a text field for the final total score.
4. No automatic calculations. Users must add their scores manually and type them in.
5. The PDF must be saveable with the user's entries so they can email it back to me.
6. Fields should have a logical tab order so users can move through the form easily.
7. The layout should remain exactly like my Word Document - do not redesign it.
8. The final result should look clean and professional, like a consultant's worksheet.
9. It will also need 6 text fields at the end where they can enter their contact information.

Deliverable:
A finished fillable PDF that I can immediately send to customers.
